Kardiologie, Angeborene Herzfehler
Meine Forschungsinteressen liegen im Bereich der Erwachsenen mit angeborenem Herzfehler (ACHD), mit einem Schwerpunkt auf der klinisch-translationalen Erforschung komplexer ACHD-Phänotypen. Ziel ist es, durch präzise Diagnostik, verbesserte Risikostratifikation und individualisierte Therapieansätze die Versorgung dieser wachsenden Patientengruppe nachhaltig zu verbessern.
• Transposition der großen Arterien (d-TGA)
Fokus auf Reinterventionen nach arterieller Switch-Operation (ASO), insbesondere im RVOT-Bereich, sowie auf späte Komplikationen der reimplantierten Koronararterien. Als Mitkoordinator der europäischen EPOCH-ASO-Kohorteuntersuche ich funktionelle und prognostische Verläufe. Ein begleitendes translationales Projekt am IBiS Sevilla befasst sich mit microRNAs und endothelialer Dysfunktion.
• Fallot-Tetralogie mit pulmonaler Insuffizienz
Analyse des Zeitpunkts der Pulmonalklappenimplantation (PVR) bei asymptomatischen Patient:innen. Ziel ist eine evidenzbasierte Entscheidungsfindung basierend auf funktionellen Parametern aus multizentrischen MRT-Daten und retrospektiven Kohortenstudien.
• Systemischer rechter Ventrikel (sRV)
Langzeitprognose und Therapie bei Patient:innen mit Mustard-/Senning-Operation oder ccTGA. Mitkoordination der multizentrischen SERVE-RCT zur PDE-5-Hemmung mit Tadalafil sowie Studien zu prognostischen Biomarkern und klinischen Komplikationen.
• COVID-19 bei ACHD-Patient:innen
Koordination des internationalen ACHD-COVID-19-Trackers zur Analyse klinischer Verläufe und Risikofaktoren bei SARS-CoV-2-Infektionen. Die Ergebnisse aus mehreren Kohortenstudien (Heart 2021, Open Heart 2021, Int J Cardiol CHD 2023, JACC Adv 2023, JACC 2024) belegen die Bedeutung von frühzeitiger Risikostratifikation bei ACHD-Patient:innen während globaler Gesundheitskrisen.

European Collaboration for Prospective Outcome Research in Congenital Heart Disease (EPOCH)

EPOCH is an international multicenter registry embedded in the SACHER network, designed to evaluate long-term outcomes in adults with congenital heart disease (ACHD): www.sacher-registry.com/epoch.
I serve as a Co-Investigator for the EPOCH-ASO subproject, which focuses on the prospective follow-up of patients after an arterial switch operation (ASO). This subproject is ongoing and coordinated in Switzerland by Prof. Daniel Tobler, Prof. Matthias Greutmann, and myself.
In addition to EPOCH-ASO, the EPOCH collaboration previously included the COVID-19 ACHD Tracker, which investigated outcomes of SARS-CoV-2 infection in ACHD patients. The findings were published in International Journal of Cardiology (PMID: 33685931).

Effect of phosphodiesterase-5 inhibition on SystEmic Right VEntricular size and function – A multicentre, double-blind, randomized, placebo-controlled trial – SERVE

SERVE was a double-blind, randomized, placebo-controlled, multicenter superiority trial (NCT03049540) conducted in Switzerland. The study enrolled 100 adults with a systemic right ventricle (sRV), comparing tadalafil 20 mg once daily versus placebo over a 3-year period. More information: www.sacher-registry.com/serve
The primary endpoint was the change in right ventricular end-systolic volume (RVESV); secondary endpoints included changes in right ventricular ejection fraction, NT-proBNP levels, and exercise capacity.
I was involved as a Co-Investigator at the Basel and Bern center, contributing to patient enrollment, follow-up, and data analysis.

SACHER – Swiss Adult Congenital HEart Disease Registry
Research Project | 1 Project Members
SACHER is a prospective national registry for adults with congenital heart disease (ACHD) in Switzerland. It systematically collects clinical, procedural, and long-term outcome data from all major Swiss ACHD centers. The aim is to support observational research, quality benchmarking, and evidence generation for this growing patient population. More information is available at: www.sacher-registry.com.
I am involved as a Co-Investigator, contributing to patient enrollment, data quality control, and scientific analysis at the Basel center. The registry serves as a platform for multiple subprojects and collaborations, including EPOCH and EPOCH-ASO.
